Step 5: Ending The Session

On the next screen, show the patient the Next Appointment Toolkit booklet and explain that it can help them prepare for their next visit.

Make sure the patient leaves with their completed goal and action plan. This includes a QR code to this website where they can access RETURN at home, in their own time.

Before the patient leaves, offer encouragement.

You might say something like: “You’ve set a clear goal today — that’s a real achievement, and I’m confident you can follow through with it.
