Step 4: making realistic goals and plans

Large or vague goals (for example, “I’ll go to the dentist soon”) can feel overwhelming.

By breaking goals down into smaller, realistic steps that fit the patient’s life, change can feel more achievable.

Talk with the patient about:

  • What their week or month looks like
  • What feels manageable right now
  • What might get in the way
